This applies to every business that ceased operation in order to obtain Clearance or closure certification from Business Permit and Licensing Office. Termination shall mean that the business operations are stopped completely.

Checklist of Requirements


Duly accomplished retirement application form originally signed by the owner/president or vice president

Original Business Permit (latest). 

If single proprietorship: Original Affidavit of closure with exact effectivity date of closure.  

If partnership: Original Partnership Dissolution with exact date of closure (originally signed by all partners.)  

If corporation: Original Secretary's Certificate or Board Resolution on closure or transfer of business with exact effectivity date of closure. 

Valid I.D. 

Original Barangay Certificate with exact effectivity date of closure.  

Original Certified Breakdown of Sales, if there are two or more lines of businesses/branches & if AFS is consolidated.

Process

Secure queuing number from Information Desk at the lobby and proceed to the Business Permit and Licensing Office waiting area and wait for the number to be called in the monitor 

Step 1

Submit the complete requirements and secure the order of payment at the BPLO Counters 9-13

Agency Action :
1.1 Call the number issued to the client 

1.2 Check the requirements 

1.3 Issue the order of payment

Computation of Business Tax and  other Fees  are based on the on the submitted Certified breakdown of gross sales or receipts

Step 2

Secure a queuing number from Information’s Desk at the lobby and proceed to the Municipal Treasurers Office for payment and return to BPLO Counter 9-13 for the Certification

Agency Action :
2.1 Call the number issued to the client 

2.2 Issue the Official Receipt 

2.3 Check the validity of the Official Receipt and instruct the client to sit at the lobby or at the BPLO designated waiting area and wait for his/her name to be called  

2.4 Print the Certification of Business Retirement for BPLO Head signature
